À Francfort, au petit matin, Elvira, une transsexuelle autrefois prénommée Erwin, est corrigée par un groupe d'homosexuels. De retour chez elle, elle retrouve son ami Christoph Hacker, un comédien sur le déclin. Après une violente dispute, celui-ci la quitte définitivement. Elvira tente en vain de l'en empêcher avant de se confier à une prostituée, Zora. Dans un hôtel, elle lui raconte son enfance dans un orphelinat, son apprentissage de boucher, son mariage avec Irène, la fille de son patron, sa rencontre avec le trafiquant Anton Saitz pour qui Erwin est devenu Elvira, son arrestation et son incarcération…
